Supporting patients with long-term health conditions to self-manage their health can improve clinical outcomes and can lead to more cost-effective use of health services.
Unfortunately, demands on the system ultimately mean that the approach taken to provide self-management is not as patient-centred as patients need.
Our use of the Patient Activation Measure (PAM) tool triages patients to interventions which suit them better. We support them to live a better life with their long-term condition.
