The Self-Management for Life "Personalised Care Planning Programme – having meaningful conversations" has been developed to better support staff helping people with long-term health conditions.

The aim of the programme is to enable healthcare professionals (HCPs) to confidently apply a personalised approach within their practice.

The meaning of ‘Personalisation’ and to understand why this approach is beneficial, particularly for people living with a long-term health condition is explored and HCPs will develop a ‘real’ understanding of what it’s like to live with a long-term health condition.

HCPs will learn to identify specific skills which support a personalised approach and reflect on those skills which they already apply as well as become more aware of the way they communicate and the impact of communication in care planning. Increase confidence in dealing with people with long-term conditions.
