Over a thousand Leeds residents who live with a long-term condition will be able to access FREE self-management training and support in a number of community events organised for September and October.
self management uk, a charity leading provider of self-management training and education in the UK, is working with Leeds City Council (Public Health) and the NHS to deliver a self-management training and awareness programme, which will help up to 1200 people across NHS Leeds North, NHS Leeds West and NHS Leeds South and East Clinical Commissioning Groups.
The programme will deliver a number of self-management training events over September and October, including a specific training session for people living with diabetes. All events are FREE for adults who have any long-term conditions, such as diabetes, heart disease, lung disease, stroke, persistent pain or other medical conditions.
Participants will learn self-management tools, skills and techniques to help them manage their condition better, improving their health and wellbeing. Events will be held in community venues, with the presence of local charities and organisations, giving participants a chance to find out about other support available to them.
self management uk is working with healthcare professionals and community organisations to encourage as many people as possible to attend and learn more about how self-management can make a positive impact on their lives. By attending these events participants will:
Jacqui Pollock, patient and stakeholder engagement manager, self management uk, said: “Self-management training is crucial to help individuals feel empowered and knowledgeable about their health. We want to reach as many people as possible and help them build their confidence and skills to improve their health and wellbeing.”
Dr Ian Cameron, Leeds City Council Director of Public Health, said: “There are over 145 thousand[1] adults living with long-term conditions (including cardio vascular disease, respiratory problems and diabetes) in Leeds. This figure is set to rise over the next 10 years.
“Leeds City Council is committed to making sure people are involved in decisions made about them through supportive self-management.”
These events are part of a 9-month pilot that will inform the three Clinical Commissioning Groups and Leeds City Council on the development of future commissioning of structured self-management education programmes for adults.
