Enhancing quality of life for people with long-term conditions and ensuring people feel supported to manage their condition are key requirements included in the NHS Outcomes Framework. Active support for self management is the King’s Fund number 1 priority for commissioners.
We can help you design and implement self management solutions that deliver on the Quality, Innovation, Productivity and Prevention (QIPP) challenge by improving care to patients and contributing to NHS savings.
