Our Vision Our vision is a society in which everyone who lives with one or more long-term conditions has access to the high quality self-management education and support that they need, enabling them to live healthier and happier lives. Our Strategic Goals design and deliver evidence-based programmes to meet the needs of specific groups of people promote the role of self-management within public health policy and delivery research and evidence the barriers and challenges people face to self-managing and what great self-management looks like to drive quality standards in outcomes for everyone Our Values & Behaviours we are person centred and are led by the experiences and views of people living with long-term health conditions we are insight driven and evidence based in our focus and decision making we are ambitious and strive for new ways to overcome barriers and challenges to achieving our purpose we speak-up and speak out when we need to "I know without the group I definitely wouldn’t be where I am today. Self Management UK has for many years wanted to draw more learning from the depth of our experience collected over many years. We are keen to work with funders and receive grants to allow us to: Research and evidence the barriers and challenges people face to self-managing and what great self-management looks like to drive quality standards in outcomes for everyone Establish a grant funded research programme focussed on unmet need, inequalities and programme effectiveness Explore the impact of person-centred care at local levels and contrast with National policy Develop a volunteer support programme to enable volunteers to be effectively recruited, trained and supported Funds received We have received funding from Guys and St Thomas’ Charity and the School for Social Entrepreneurs as part of their Health & Wellbeing Trade up Programme. The grant is allowing us to analyse data on the impact of self-management support in the London boroughs of Southwark and Lambeth. In 2017 we received a grant from the Transform Foundation to update our IT equipment and structure as well as develop a new website.
